• Honorable Dismissal

• Official Transcript of Records (OTR)

• Certification, Authentication and Verification (CAV)

• Certificate of Good Moral Character

• PSA Birth Certificate(original)

• If Married, PSA Marriage Contract (Photocopy and Original)

• One Long Brown Envelope

C. Returnees

• If returnee, same requirements as the transferee.

• For students enrolled immediately preceding the term, with no record of academic deficiency no status of dismissal from the institution or exclusion in his last term at SIS, an unofficial checklist need to be submitted.

• For those with previous academic delinquency status, interview with the dean is required.
